The Diocese of Columbus has asked all parishes to participate in a project gathering parishioner input via an instrument called the Disciple Maker Index (DMI).

The 10–15-minute survey can be accessed through an online link through January 6 and will ask you to reflect on your own spiritual growth and enable you to provide your perspective on our parish’s efforts to help you grow in relationship with Christ. All responses will be confidential, and the parish will only receive information about the community as a whole. You can access the online survey at https://portal.catholicleaders.org/d/c6b4xc. If you prefer a paper copy, you may pick one up in the Narthex. To submit a paper survey, please drop it off in the Parish Office or place it in the offertory basket at Mass.
